1. Insurance & Payment Methods Accepted
To ensure a smooth experience, Ever Medical Spa accepts:

We do not participate in most insurance plans for aesthetic services. Payments are expected at time of service, unless otherwise arranged.
2. Estimate of Charges
At your initial consultation, our team will provide an estimate of fees based on your personalized treatment plan—whether it’s injectables, laser, skin care, PRP/PRF injections, or medical spa procedures. Please note this is only an estimate; actual costs may vary slightly depending on your unique treatment.
3. Payment Expectations

Payment is due at the time of service unless prior financing or membership arrangements are in effect.

If your procedure requires multiple sessions, payment is typically handled at each appointment. For package programs, deposits or pre-payment may be required.
4. Late Payments & Missed Appointments

Late Payments: A 1.5% monthly interest charge (18% APR) is applied to any balance 30+ days past due.

Missed or Late Arrivals:

No-show or cancelations with less than 24 hours’ notice will result in a $50 fee.

Arriving 15+ minutes late without notice may require rescheduling, and a late fee could apply.

5. Package Deals & Membership Plans
Save on combined treatments with our exclusive packages (e.g., Microneedling + PRP/PRF, Laser + Peels, Injectable bundles).

Non-transferable and valid for one year from purchase.

A non-refundable deposit of 25% of the total package price is required.

Remaining balance may be paid upfront or per session.

Unused sessions expire after 12 months.
6. Refund Policy

Cosmetic medical spa services (e.g., Botox, fillers, lasers, PRP/PRF, body contouring) are non-refundable once performed.

For multi-session packages not yet started, a refund minus deposit is available within 30 days.

Membership plans are non-refundable once the monthly process is complete.
7. Financing & Payment Plans

8. Additional Policies

Returned Checks: A $35 fee applies for returned checks.
